New Adult Membership

Use this form to enroll adult
volunteers in Girl Scouts. Paper
forms are available. Renew annually.
Can be renewed on the My Troop Tab
in the Volunteer Toolkit by troop leaders.

or submitted online.

Girl Membership

Use this form to enroll girls in Girl Scouts. Paper forms and payment returned to troop leader or council. Renew annually. Can be renewed in the Volunteer Toolkit by parents or troop leaders.

Extended-Year Girl Membership

Use this form to enroll new girls in Girl Scouts between April 1- July 31, 2026. With our Extended Year Membership for $121, you can start as early as April 2026 and keep the fun going through September 2027.

Permission Slip

Use this form to collect girls health
info and parent permission (or denial)
for basic troop activities, giving
over-the-counter meds, taking part

Use this form for parents or guardians
to authorize activities that focus on
subjects that may be considered sensitive.

Girl Release

Use this form for parents or guardians
to authorize people to pick up their
daughter from her troop meeting,
events, etc.

Permission to Sell

Use this form to get permission from their legal guardian for participating in both product programs, Girl Scout Cookies or Fall Product Program.

Troop Information Update

This form is filled out annually to provide as much information about your troop as possible. This will give accurate information for Girl Scouts looking to sign up and what the troop's focus is.

Honor Troop

The Honor Troop Awards were established by Girl Scouts
Heart of Michigan to recognize troops that make Girl Scouting an exceptional
experience, setting themselves apart as role models. This form encompasses
membership year activities and must be turned in on or before September 30.

Troop Trip/Activity	Use this form when your troop does an activity that’s not routine: the troop plans to travel outside Michigan; the activity lasts more then eight hours or overnight; the activity has a higher risk according to the Safety Activity Checkpoints.
